What is Wraparound?

Wraparound is a strengths-based, family-driven process that coordinates care around the whole child. Supports are tailored to each family to promote stability, growth, and long-term success.

CSED Wraparound

CSED Waiver Wraparound supports children and youth with serious emotional or behavioral needs by providing intensive, individualized care within the home and community. Using a strengths-based, youth-focused approach, the Wraparound Facilitator brings together the family, service providers, and natural supports to create a personalized plan of care. Services are coordinated to address emotional, behavioral, and functional needs while promoting stability, growth, and meaningful participation in daily life. The goal is to help youth thrive at home, succeed in school, and build lasting skills for the future.

SAH Wraparound

Safe at Home Wraparound is a family-centered program designed to help children remain safely in their homes and communities. Through a team-based approach, families are supported in identifying their strengths, addressing challenges, and building practical solutions that fit their unique needs. A Wraparound Facilitator works closely with the family and involved providers to coordinate services, strengthen relationships, and reduce the risk of out-of-home placement. The focus is on safety, stability, and empowering families with the tools and supports they need to succeed long-term.

Home Base, Inc. proudly serves all 55 WV counties for CSED Wraparound through telehealth.

  • If you would like HBI to provide your wraparound services for CSED reach out to your Aetna Care Manager, and they will provide you with the Freedom of Choice form for you to select your preference for wraparound services.

  • If you would like HBI to provide your wraparound services for SAH, reach out to your DoHS worker to help you start or switch your wraparound services.  

My name is Tiffany Timmerman

Wraparound Program Director, BA in Psychology

As the Wraparound Program Director, it is my responsibility to facilitate all WV Wraparound Programs for Home Base Inc. across the state of West Virginia. I ensure that all staff are trained and supported in their direct work with families on a daily basis. As the Wraparound Facilitation Program Director, I assign families in need to staff for Safe at Home and the Children with Serious Emotional Disorders Waiver program for Wraparound Services. I have been with Home Base Inc. for over two and a half years, starting as a Wraparound Facilitator, then being promoted to a Lead Wraparound Facilitator, a Wraparound Facilitation Program Supervisor and now the Wraparound Facilitation Program Director. Prior to my work at Home Base Inc., I worked in Child Welfare for 10 years, conducting Child Protective Services Investigations, Intake for the Abuse and Neglect Hotline, Child Protective Services Ongoing Services and Family Preservation Services.

The West Virginia Family Resource Networks are organizations that respond to the needs and opportunities of the community. Partnering with community members and public and private organizations, the WVFRN members develop innovative projects and provide needed resources for their local areas.
